Use the Internet and/or the online library to access an article describing a medical error. In this two to three page paper, make sure you use APA format and at least two sources.
Determine if the error was a result of action versus inaction. Explain your decision.
Determine if the errors was skill based slips and lapses; rule-based mistakes; knowledge-based mistakes; and violations of norms and rules. If the article does not divulge this information, pick one for sake of the paper.
Describe one would use the FMEA process to analyze the error and to recommend improvement strategies.
Examples of medical errors include:
Surgical events (wrong body part, wrong patient, wrong procedure, retention of foreign object, intra-op/post-op death of a normal, healthy patient)
Death or serious disability from product/device
Infant discharged to the wrong person
Suicide or attempted suicide while being cared for in a healthcare facility
Death or serious disability associated with a hemolytic reaction during a blood transfusion
Maternal death or serious disability associated with labor or delivery on a low-risk pregnancy while being cared for in a healthcare facility
Patient death associated with a fall while being cared for in a healthcare facility
Patient death or serious disability associated with the use of restraints or bedrails while being cared for in a healthcare facility
